Slide the gnome into the back window of the car (hat first) and just release him (not throw! just release) when he's about halfway through the rear window. Pick the gnome up and approach the back of the car, walk to its left, the physics of the game should automatically angle the gnome on its side, just keep trying if it doesn't.
